The following page will outline your standard daily operations structure of items that will need to be captured while working on AT&T projects across

the United States. The following specifications will guide you through completing the Closeout Documentation Package (DELIVERABLES) for your

projects while using our “Website/WebApp”  = http://telc.telecomcloseouts.com, called: 

                       NOTE: All Structure & Antenna info in each document must match the RFDS, CD, RETs, AISG, OSAC, PHOTOS, SWEEPS,

                                 PIM TEST, FIBER TEST, AS-BUILT’S / REDLINE DRAWINGS & TAPE DROP documents accurately.

Document Type Guide :

Standard Document Naming Conventions for all markets:  (Project Initiaitve.FA#.DOCID.yymmdd)

                         Example: As-Built / Redline Drawings = ( LTE2C.10010761.AE202.150506 )

Submit the latest rev ‘O’ “Issued for Construction” or “Final “drawings - Signed & Dated to include:

Your Name,

Company Name,

Date on every page,

Drawings marked “Built as Drawn” or “Built per Print”, etc. will be rejected.

Redline drawings must show all changes not built as drawn & reflected on all other applicable closeout documents.

AN201 - Antenna Azimuth Tool Results

Azimuth Reporting Requirements:

 

Sunsight Instrument use requirements:

Submit a summary report for all applicable azimuth measurements.

Submit photos of the azimuth measuring instrument as a whole, showing how it is mounted to each applicable antenna

with a visible label of the appropriate Sector & Site # marked on the antenna.

3Z RF ALIGNER Instrument use requirements:

Submit screenshots of each applicable azimuth measurement.

Submit photos of the azimuth-measuring instrument as a whole, showing how it is mounted to each applicable antenna

with a visible label of the appropriate Sector & Site # marked on the antenna.

RET Form Worksheet - Furnish all requested info. All info must match associated closeout documents. ALD Serial # must match

what is on the RET Screenshots. Report installer name & date.

DE107 - Site Photos

Closeout Photos:

Any photos that are in addition to the listed required site photos.

Photo Naming Convention/Labeling:

Required Site Photos – Project Initiative.FA#.PhotoID#.yymmdd

All hardware tight & marked (ground wire should be labeled both ends)

NOTES:

All photos must have Time/Date stamp.

RAD Centerline must be marked with sharpie on back of the antenna.

Sharpie Markings must be visible/legible on all Antenna.

       AN501 Downtilt & Plumb Photo requirements for installed or moved antennas on latest ND298.

Furnish an AN501 Downtilt photo clearly showing the instrument on the back of the antenna.

Furnish an AN501 Plumb photo of the same antenna clearly showing the instrument on the side of the antenna.

RET Screenshots (Not required for RRH only installations)  – Must have Alpha, Beta & Gamma antenna serial #, model #,

site #, sector, status showing “ok”, low and high frequency.  All info must match associated closeout documents.

NOTE: Alternatively you can use the POST CCU REPORT and import it to create the entire RET FORM.

DL201 - Asset Movement Form

DL201 - Asset Movement Form (AMF): This Document is for Asset Tracking. Normally used for Equipment being moved

from one site to another site only. Also can be used for Decommissioned Equipment being removed from the site.

Submit AMF form for all sites listing any equipment removed from the site. Include antenna model & serial #s

o

Report site # & name

o

If nothing was removed, state “Nothing removed from site” on the form

DL202 - Equipment Release Authorization Form

ERF Forms - Will be provided from the Equipment Engineers of AT&T. This is for all AT&T Ordered items that are to be provided

for the project. The Turf Vendor may provide a secondary / additional ERF / Pick List of equipment they had to order for the project

but the primary ERF Form will always be of equipment provided directly by AT&T Equipment Engineers and can be picked up

at the AT&T CSSL / Warehouse / XDock unless the items have been already picked up by the Turf Vendor. 

PM201 - (RFDS) Radio Frequency Data Sheet-“Redlined”

Redlined RFDS:

o

Submit the latest revision used for construction – should say “As-Built/In Progress” top right corner on front page.

o

Ensure that the PTN # matches the PTN # on your PO.

o

Sign & Date each and every page to include:

Your Name,

Company Name.

Date

o

Mechanical Down tilts and RAD Center must match the closeout photos.

o

Redlined RFDS must show all changes & accurately reflect all changes on all other applicable closeout documents.

PM204 - Operations Site Acceptance Checklist (OSAC)

PM204 - Operations Site Acceptance Checklist (OSAC): AT&T 502

•Complete Site Identifier Tab - all information NOTE: Construction Coordinator is the FCM, Inspection Company is Turf Vendor.

•Complete Site Checklist Tab

Report all field measured Antenna Azimuths per latest revised format. Must match Azimuth screen shots or Azimuth Summary.

Report all Mechanical Down tilts per photo measurement.

Report all RAD Centerline Heights - per photo

Report all installed equipment - manufacturer & model #s – per photo if applicable

All items on the checklist must be addressed in the “Within Spec/Drawings?” column.

PM218 - ( NFSD ) National Field Service Desk Form

PM218 - NFSD: (National Field Service Desk Form)

Report all site entries & exits to include:

Ticket #s,

EIM Ticket Number as required,

AT&T Techs Name,

o

AT&T Techs ATTUID #, (this is his or her AT&T Employee email address).

RG114 - Tape Drop Form

RG114 - Tape Drop (Review “Mod Instruction” Tab): (Not required for RRH only installations.)

Complete all top header info. All requested info can be found on the RFDS: 

              Date,

Site Name,

Market,

FA Location Code,

USID,

Cell Site ID, etc.

Complete Structure Type Section

o

Complete Structure Height Info Section. Must have confirming photos.

o

Complete Antenna Info (TALLEST ANTENNA TIP HT FT. AGL) section – Tip heights to match photos.

o

Complete Marking & Lighting Info section. Reference Green sheet/Guardian and/or RFDS (page 1/section 4).

o

Complete signature section

o

Complete PCS/Cellular/LTE Antenna Detail: Antenna Tip Heights, Antenna Model #s & Project Initiative per Antenna

              Sector Position. Must have confirming photos.

TS201 - Sweep Test Results

TS201 - Sweeps: (Not required for RRH only installations.)

o

Submit sweeps in pdf format (2 or 3 Plot *.PDF files),

o

Do not submit a failing Sweep or multiple pages, must be (1) *.Pdf file only with all sweeps included in it.

o

Required after any UMTS/GSM antenna disconnection/re-location.

o

Required for UMTS and/or GSM antennas.

o

Low & High band sweeps are required.

o

Required for installation/relocation of TMAs.

TS202 - PIM Test Results

TS202 - PIM Tests:

1C & 3C – High & Low Tests & Graphs required. In just (1) *.Pdf file.

2C – Frequency that is being installed (High or Low) Tests & Graphs required. In just (1) consolidated *.Pdf file.

•Submit test reports on all 6 ports of each antenna for a total of up to 18 reports

1D65C antenna Hi Band-12 tests

1D65C antenna Low Band-6 tests

•Submit test reports on all 4 ports of each antenna for a total of up to 12 reports

1D6565C antenna Hi band-6 tests

1D6565C antenna Low band-6 tests

•Submit graphs for all 6 ports of each antenna for a total of up to 18 reports

1D65C antenna Hi Graph - 12 graphs

1D65C antenna Low Graph - 6 graphs

•Submit graphs for all 4 ports of each antenna for a total of up to 12 reports

1D6565C antenna Hi Graph - 6 graphs

1D6565C antenna Low Graph - 6 graphs

•All tests must be “passing”.

•Test Point Description should include color coding and serial # of antenna.

•Antenna serial #s to match AISG/RET Screenshots antenna serial #s.

TS203 - Fiber Test Results

TS203 - Fiber Tests:

 

TS52 Driver

•Each test consists of an 18 pair cable and all Remote Radio Head fiber jumpers in project at time of deployment.

•Certify entire Fiber Trunk

•A full system from ground to RRU which includes jumper and squid to RRU with images.

•Must have 2 images per fiber pair

•Must have Readouts/Meter log for all 18-fiber tests including jumper testing.

•Must combine in one PDF & site name.

•Create just (1) *.Pdf file and include all Fiber Test Results.

TS53 Driver

•Used where main fiber trunk exists or RRH’s on ground that do not use main fiber trunk.

•Each test consists of up to 6 fiber jumpers.

•Test pairs being used only.

•Must test through Fiber cable bottom connection.

•A full system from ground to RRU which includes jumper and squid to RRU with images.

•Must have 2 images per fiber pair

•Must have Readouts/Meter log for all fiber tests including jumper testing.

•Must combine in one consolidated PDF & Site name.
